NAACLS / CAAHEP / ABHES Accredited Programs
If you are currently enrolled in or have completed a NAACLS / CAAHEP / ABHES accredited training program within the last five years, the printed application form must include the program director's name and the school code number must be indicated.
The education received from a NAACLS / CAAHEP / ABHES accredited program is acceptable for a period of five (5) years from the date of completion of that program. After five years, the applicant’s eligibility will be based on clinical laboratory experience (except Cytotechnologist) as stated in the current examination eligibility requirements.

Military trained MLT applicants
Official or notarized documentation of your military code (NEC, MOS or AFSC code or ER.B), certificate verifying completion of training or letter from Program Director documenting completion date or notarized copy of your DD214 form must be submitted with your application form. Structured Laboratory Training Programs
If you have successfully completed a structured program under the auspices of a NAACLS accredited Medical Laboratory Science Program:
Step 1: Please download the appropriate documentation form. These forms refer to the particular eligibility requirement for your certification category. If your eligibility route is not listed below, a training documentation form is not required.
Donor Phlebotomy Technician
If you are RN or LPN, you must submit a notarized copy of your current state or provincial license along with your application form.
Phlebotomy Technician Training
- Route 2: Download training documentation form (PDF). The training documentation form must be completed by your program director and attached to a Letter of Authenticity*, on letterhead, signed by the program director verifying the accuracy of the information on the form.
- Route 4: Download training documentation form (PDF). The training documentation form must be completed by the program director of the accredited allied health program you completed. You must include a Letter of Authenticity*. You must submit a notarized copy of your current state/provincial license for RN or LPN or a notarized copy of certificate of completion from the accredited allied health program you completed along with your application form.
- Route 6: Download experience documentation form (PDF). The experience documentation form must be completed by an appropriately qualified supervisor (certified medical laboratory scientist, licensed nurse or other licensed/certified health care practitioner). You must include a Letter of Authenticity*.
- Route 7: If you are applying from a California Dept. of Health Services approved program download training documentation form (PDF). The training documentation form must be completed by your program official and attached to a Letter of Authenticity*, on letterhead, signed by the program official verifying the accuracy of the information on the form.
*Letter of Authenticity must be submitted from your program official with the appropriate training documentation form. The Letter of Authenticity must be printed on original letterhead, state that the reference form was completed by your program official and include the date and your program official’s signature.
